Job DetailsConnect individuals overcoming addiction with continued support to help them on the road to recovery!Join our team of competent and compassionate professionals as the After Care Coordinator in the New View program at Behavioral Health Network!Your experience, knowledge and great skills will support the New View program with the following:
Coordinating and facilitating referrals to community-based supports based on the individuals' needs, which may include next level of care, MAT, primary care, follow-up appointment, housing, entitlements, etc.
Providing information, referral, and follow-up as specified in the treatment plan for individuals served.
Working with treatment team and the families of individuals served to ensure successful referral to and engagement in next level of care.
Facilitating diverse treatment groups, as needed.
Completing program documentation regarding individuals served in a timely and thorough manner.
Recognizing and reporting any concerns, contacting a supervisor for guidance and/or in an emergency, when applicable.
Who are we?The New View program at BHN serves women who have been designated for substance use disorder treatment via the legal system.
The program offers acute treatment (ATS) and crisis stabilization (CSS) services.

High School Diploma or GED equivalent (required); Bachelor's Degree in a related field (preferred).
At least one year experience working in SUD population (required).
Excellent verbal and written communication skills.
Familiarity with internet, word processing and email systems.
Familiarity with DPH BSAS regulations and referral sources (preferred).
At least two years of sobriety, if applicable (preferred).
